About This Depression Screening Questionnaire (PHQ-9)

This questionnaire is a brief mental health screening tool commonly used by healthcare professionals to help identify symptoms associated with depression.

The PHQ-9 consists of 9 questions about mood, energy, sleep, concentration, motivation, and daily functioning over the past 2 weeks.
